What do I need to bring to an appointment?

Can you be our primary care provider?

The providers are trained in primary care. We will be able to handle most all of your primary care needs. However, we do require that you maintain your primary care doctor for routine screening exams such as mammograms, colonoscopies, prostate exams, etc. and for emergency situations requiring immediate care or hospitalization. We will work with you closely as consultants and coaches in preventive, nutritional and functional medicine to help you address the root of chronic health problems and can confer with your primary care provider if required.

Do you accept insurance?

We do not accept any form of medical insurance or Medicare/Medicaid. However, the office will provide you with an itemized bill with traditional ICD-10 codes and CPT codes upon request.
 You may also use pre-tax dollars from a Health Savings Account (HSA) to pay for your consultations and follow up visits.

Will my medical insurance reimburse me for my visits?

We will provide you with your diagnosis and test codes and a receipt so that you may file for reimbursement with your insurance provider if you choose to do so.  We have found that some insurance companies request additional information that is very specific and specialized to that provider. Given the large number of discrepancies between what each company requires and the time needed to meet those requests, we do not assist with insurance reimbursement above and beyond the diagnosis, codes and receipt given at check out.

Will my medical insurance pay for lab testing?

We offer two types of lab testing: conventional lab testing and functional lab testing: conventional labs are those that you are likely already familiar with. These are the labs such as thyroid testing, vitamin D and cholesterol, to name a few. Integrative Health and Wellness is typically different from most conventional practices in that we may often order more or unique lab testing that is often interpreted much differently than conventional practice. We listen to your symptoms while looking at optimal levels rather than normal ranges. Most of our conventional labs are done through South Bend Medical Foundation and they will take your insurance information and file for you. We are currently using Genova Diagnostics for our functional lab testing. If ordered, you will receive a test kit at your appointment with a requisition form to complete as well as providing your insurance information for billing. Genova will file with your insurance company.

Do you prescribe medicine?

Our providers can, and do, prescribe medications whenever necessary. They are fully licensed and Board-certified to do so.
